                  आधार कार्ड की तरह सरकार ने हेल्थ आईडी कार्ड लॉन्च किया है। इसके माध्यम से भारत भर में सभी स्वास्थ्य सेवाओं से कार्ड धारक निर्बाध रूप से जोड़ा जाएगा।

हेल्थ कार्ड क्या आवश्यक्ता है?

हेल्थ आईडी का उपयोग करना आपके और आपके परिवार के लिए सुरक्षित डिजिटल स्वास्थ्य रिकॉर्ड बनाने की दिशा में पहला कदम है। यह कार्ड आपको भाग लेने वाले स्वास्थ्य सेवा प्रदाताओं और भुगतानकर्ताओं के साथ अपनी सहमति से अपने स्वास्थ्य डेटा तक पहुंचने और साझा करने की अनुमति देता है। यह आपको और आपके डॉक्टर को और बीमा कंपनी जैसे हितधारकों को आपकी पूरी जानकारी पाने में मदद करेगा; अर्थात आपके :

• अस्पतालों में प्रवेश की जानकारी दे देगा
• उपचार की जानकारी दे देगा और
• छुट्टीहोने की जानकारी दे देगा

हेल्थ आईडी कार्ड धारक को कैसे लाभ होता है?

आपात स्थिति में घर के लोग या बहरी मददगार लोग मरीज को बिना उसकी रिपोर्ट और दवाइयों के पर्चे लिये ही अस्पताल ले जाते हैं। ऐसा होना भी चाहिए – जान बचाना जरूरी है न कि रिपोर्ट जुटाना। इसलिए, डॉक्टर को रोगी के मामले पर नए सिरे से विचार करना होता है। इसकी वजह से:

1. इलाज शुरू होने में देरी होती है क्योंकि डॉक्टर रिपोर्ट आने का इंतजार करते हैं।

2. व्यक्ति का खर्च और दर्द बढ़ जाता है क्योंकि उसे उन परीक्षाओं को भी करवाना पड़ता है जिनके लिए उसकी पहले जांच की जा चुकी है, और शरीर को झूठ मूठ में सुई कोचवाना पड़ता है और X -ray की तरंगें शरीर में ले करके नुक्सान उठाना पड़ता है।

यदि डॉक्टर को स्वास्थ्य कार्ड तक पहुंच प्रदान की जाती है, तो वह –

1. पहले हुए उपचारों के बारे में तुरन्त जान जायेंगे, जिससे उनको त्वरित निर्णय लेने में मदद मिलेगी।

2. मान लीजिए कि व्यक्ति कोमा में चला गया है या बोलने में सक्षम नहीं है तो नामांकित व्यक्ति स्वास्थ्य कार्ड का उपयोग कर सकता है और डॉक्टर को आवश्यक विभिन्न विवरण बता सकता है।

3. यदि कार्ड धारक को कुछ दवाओं से एलर्जी है, तो उसका भी उल्लेख हेल्थ कार्ड में होगा उसके वजह से डॉक्टर यह जान जायेंगे कि किन दवाओं को नहीं देना चाहिए।

4. बच्चे अपनी परेशानी बता नहीं पाते हैं – केवल रोते हैं. बच्चों के स्वास्थ्य कार्ड को देखकर डॉक्टर समझ जायेंगे की पहले कैसे उपचारों से बच्चा ठीक हो जाता था. उसी लाइन पर नये डॉक्टर भी बढ़ चलेंगे. अभिप्राय यह हुआ कि यदि फैमिली डॉक्टर नहीं हैं तो यह कार्ड फैमिली डॉक्टर की तरह ही दुसरे डॉक्टर को भी ग्यानी बना देगा और आपको उनकी कमी महसूस नहीं होगी.

डिजिटल हेल्थ रिकॉर्ड से बीमा कंपनी को कैसे होता है फायदा?

यह सर्वविदित है कि कई लोग फर्जी प्रवेश दिखा करके बीमा सेवाओं का दुरुपयोग कर रहे हैं – गलत क्लेम ले रहे हैं। इस कार्ड के माध्यम से गलत दावों का पकड़ में आना आसान हो जाएगा एवं फर्जी भुगतान बंद हो जाएगा। जब कम भुगतान करना होगा तब बीमा कंपनियां अपने प्रीमियम की दर कम कर सकती हैं अतः ऐसा संभव है कि निकट भविष्य में हेल्थ कार्ड से आम जनता की मदद कम प्रीमियम के रूप[ में हो जाये ।

आसान साइन अप

ओटीपी प्राप्त करने के लिए आपके पास बस अपना आधार नंबर और मोबाइल होना चाहिए।

अगर आपके पास आधार नहीं भी है तो भी आप केवल मोबाइल नंबर से भी साइन अप कर सकते हैं।

आसान ऑप्ट आउट विकल्प

आप कभी भी अपने डेटा को मिटाने का अनुरोध कर सकते हैं।

स्वैच्छिक ऑप्ट-इन

हेल्थ कार्ड बनवाना जरूरी नहीं है। यदि आप लाभ उठाना चाहते हैं तो आप लाभ उठा सकते हैं। नहीं तो कोई बात नहीं।

सहमति आधारित पहुंच

आपकी स्पष्ट और सूचित सहमति के बाद ही आपके स्वास्थ्य डेटा तक किसी की पहुंच प्रदान की जाती है।

आप जब चाहें तब आप अपने सहमति को प्रबंधित करने और रद्द करने की क्षमता रखते हैं।

सुरक्षित और निजी

इसमें सुरक्षित जानकारी को कोई और नहीं पा सकटा है क्योंकि यह सुरक्षित और एन्क्रिप्शन तंत्र के साथ निर्मित हैं और यह सरकारी खर्च पर आपकी निजी लाकर है क्योंकि आपकी सहमति के बिना कोई भी जानकारी साझा नहीं की जाती है।

समावेशी पहुंच

इसको कोई भी बनवा सकता है भले ही उसके पास स्स्मार्ट फोन हो या बटन वाला फोन हो या फोन ही न हो.

एबीडीएम क्या है?

ABDM अर्थात आयुष्मान भारत डिजिटल मिशन । इसका उद्देश्य देश के एकीकृत डिजिटल स्वास्थ्य बुनियादी ढांचे का समर्थन करने के लिए आवश्यक रीढ़ की हड्डी का विकास करना है। यह डिजिटल राजमार्गों के माध्यम से हेल्थकेयर पारिस्थितिकी तंत्र के विभिन्न हितधारकों के बीच मौजूदा अंतर को पाटेगा।

पीएचआर क्या है?

पी एच आर का अर्थ है Personal Health Record यह एक व्यक्तिगत स्वास्थ्य रिकॉर्ड, या , एक इलेक्ट्रॉनिक एप्लिकेशन है जिसके माध्यम से रोगी एक निजी, सुरक्षित और गोपनीय वातावरण में अपनी स्वास्थ्य जानकारी (और अन्य जिनके लिए वे अधिकृत हैं) को बना और प्रबंधित कर सकते हैं।

मैं अपने आवेदन/फोन पर अस्पताल से अपने स्वास्थ्य रिकॉर्ड कैसे प्राप्त कर सकता हूं?

• उसके लिये आपको अपने मोबाइल पर संबंधित मोबाइल एप्लिकेशन को डाउनलोड करना होगा एवं
• स्वास्थ्य सुविधा को इनटरनेट पार खोजकर अपने एप्लीकेशन से उसको जोड़कर
• डाउनलोड करना होगा
• इस तरह से बहुत ही आराम से अपने स्वास्थ्य रिकॉर्ड प्राप्त कर सकते हैं।

क्या मैं अपने स्वास्थ्य रिकॉर्ड को स्वास्थ्य रिकॉर्ड एप्लिकेशन में मैन्युअल रूप से अपलोड कर सकता हूं?

हां, उपयोगकर्ता अपनी स्कैन की गई रिपोर्ट स्वास्थ्य रिकॉर्ड (पीएचआर एप्लिकेशन) के माध्यम से एबीडीएम के स्वास्थ्य लॉकर पर अपलोड कर सकते हैं।

क्या मेरी सभी स्वास्थ्य संबंधी जानकारी स्वास्थ्य रिकॉर्ड आवेदन में जा रही है?

नहीं, केवल डॉक्टरों और उपयोगकर्ता द्वारा बनाए और अपलोड किए गए रिकॉर्ड ही मोबाइल एप्लिकेशन पर होंगे।

क्या एप्लिकेशन पर अपलोड किया गया डेटा सुरक्षित है?

रोगी सभी डेटा का मालिक होगा, उसके पास इसे प्रबंधित करने के सभी अधिकार होंगे। एबीडीएम के साथ एकीकृत स्वास्थ्य लॉकरों का उपयोग स्वास्थ्य रिकॉर्ड को लॉकरों में सुरक्षित रखने के लिए भी किया जा सकता है।

क्या मेरे स्वास्थ्य देखभाल प्रदाता मेरी स्वास्थ्य देखभाल में समन्वय स्थापित करने के लिए मेरे पीएचआर तक पहुंच सकते हैं?

हां, लेकिन उपयोगकर्ता से सहमति लेने के बाद ही, उपयोगकर्ता की सहमति के बिना कोई भी स्वास्थ्य रिकॉर्ड तक नहीं पहुंच सकता है .

 पीएचआर पता क्या है?

PHR (व्यक्तिगत स्वास्थ्य रिकॉर्ड) पता एक स्व-घोषित उपयोगकर्ता नाम है जिसे स्वास्थ्य सूचना विनिमय और सहमति प्रबंधक (HIE-CM) में साइन इन करने की आवश्यकता होती है। डेटा साझाकरण को सक्षम करने के लिए प्रत्येक स्वास्थ्य आईडी को एक सहमति प्रबंधक से लिंकेज की आवश्यकता होगी। वर्तमान में, सभी हेल्थ आईडी उपयोगकर्ता हेल्थ आईडी साइन अप के दौरान अपना स्वयं का पीएचआर पता उत्पन्न कर सकते हैं।

 क्या मैं एक स्वास्थ्य रिकॉर्ड एप्लिकेशन के माध्यम से कई पीएचआर पता बना सकता हूं?

हाँ, एक मोबाइल नंबर का उपयोग करके एक से अधिक PHR पता बनाया जा सकता है।

HIE – CM क्या है?

स्वास्थ्य सूचना विनिमय और सहमति प्रबंधक (HIE-CM) सहमति प्रबंधक को संदर्भित करता है जो उपयोगकर्ता के लिए सहमति प्रबंधन, और व्यक्तिगत स्वास्थ्य रिकॉर्ड को साझा करने और जोड़ने में सक्षम बनाता है। ABDM ने अपना स्वयं का HIE-CM (स्वास्थ्य रिकॉर्ड्स एप्लिकेशन) शुरू किया है। आप HIE-CM (हेल्थ रिकॉर्ड्स एप्लीकेशन) पर साइन अप करने के लिए अपनी हेल्थ आईडी का उपयोग कर सकते हैं। निकट भविष्य में रोगियों को चुनने के लिए एक से अधिक सहमति प्रबंधक उपलब्ध होने की संभावना है। इस बात को में अंग्रेजी में भी लिखना उचित समझ रहा हूँ जिससे समझने में आसानी हो .

Health Information Exchange & Consent Manager (HIE-CM) refers to consent manager that enables consent management, and sharing & linking of personal health records for a user. ABDM has rolled out its own HIE-CM (Health Records Application). You can use your Health ID to sign up on the HIE-CM (Health Records Application). Multiple consent managers are likely to be available for patients to choose from in the near future.

हेल्थ आईडी के लिए पंजीकरण करने के लिए मुझे क्या-क्या चाहिए?

• मोबाइल नंबर के माध्यम से बनाने के लिए ये चीजें आवश्यक हैं – नाम, जन्म का वर्ष, लिंग, पता, मोबाइल नंबर।

• आधार के माध्यम से माध्यम से बनाने के लिए ये चीजें आवश्यक हैं – नाम, जन्म का वर्ष, लिंग, पता, मोबाइल नंबर, आधार संख्या

अर्थात दोनों ही माध्यमों से हमें नाम, जन्म का वर्ष, लिंग, पता, मोबाइल नंबर जानना जरूरी होता है; आधार नंबर देने से कई कालम खुद ब खुद भर जाते हैं।

क्या हेल्थ आईडी बनाने के लिए आधार अनिवार्य है?

नहीं, स्वास्थ्य आईडी बनाने की प्रक्रिया में आधार का स्वैच्छिक उपयोग शामिल है। यदि आप स्वास्थ्य आईडी बनाने के लिए अपने आधार का उपयोग नहीं करना चाहते हैं, तो आप स्वयं घोषित जानकारी के साथ अपने मोबाइल नंबर का उपयोग कर सकते हैं।

मेरा मोबाइल नंबर मेरे आधार नंबर से लिंक नहीं है। मैं हेल्थ आईडी बनाने के लिए अपने आधार को कैसे प्रमाणित करूं?

यदि आप हेल्थ आईडी पोर्टल पर अपना हेल्थ आईडी बनाने के लिए अपने आधार नंबर का उपयोग करते हैं, तो आपके आधार से जुड़े मोबाइल नंबर पर एक ओटीपी भेजा जाएगा। यदि आपके पास आधार से मोबाइल लिंक नहीं है, तो यह सुझाव दिया जाता है कि आप निकटतम सहभागी सुविधा पर जाएँ, और आधार संख्या का उपयोग करके बायोमेट्रिक प्रमाणीकरण का विकल्प चुनें। सफल प्रमाणीकरण के बाद, आपको सुविधा पर ही एक स्वास्थ्य आईडी प्राप्त होगी।

क्या मैं हेल्थ आईडी बनाने के लिए आधार के अलावा अन्य आईडी दस्तावेजों का उपयोग कर सकता हूं?

वर्तमान में, एबीडीएम मोबाइल या आधार के माध्यम से स्वास्थ्य आईडी निर्माण का समर्थन करता है। एबीडीएम ने जल्द ही उन सुविधाओं को रोल आउट करने का प्रस्ताव किया है जो अन्य आईडी दस्तावेजों जैसे पैन कार्ड, ड्राइविंग लाइसेंस, आदि के साथ स्वास्थ्य आईडी निर्माण का समर्थन करेंगे।

क्या मेरा स्वास्थ्य आईडी अद्वितीय है?

आपकी स्वास्थ्य आईडी आपके मोबाइल नंबर या आधार संख्या के अतिरिक्त आपके मूल विवरण का उपयोग करके बनाई गई है। आपकी स्वास्थ्य आईडी आपके लिए अद्वितीय होगी, और आपके पास अपने सभी स्वास्थ्य रिकॉर्ड को स्वास्थ्य आईडी से जोड़ने का विकल्प होगा।

क्या मैं मल्टीपल हेल्थ आईडी बना सकता हूं ताकि मैं अपनी कुछ समस्याओं को फैमिली डॉक्टर या अपने किसी परिचित से छिपा सकूं?

पंजीकरण में कितना समय लगेगा?

पूर्ण पंजीकरण में कुछ मिनट लगते हैं।

क्या मुझे कोई भौतिक दस्तावेज जमा करने होंगे?

 अभी आवश्यकता नहीं है।

क्या मेरे स्वास्थ्य रिकॉर्ड सुरक्षित हैं?

हाँ।

ABDM आपके किसी भी स्वास्थ्य रिकॉर्ड को संग्रहीत नहीं करता है। आपके स्वास्थ्य रिकॉर्ड को स्वास्थ्य सेवा सूचना प्रदाताओं के पास उनकी अवधारण नीतियों के अनुसार संग्रहीत किया जाता है, और आपकी सहमति के बाद ही एन्क्रिप्शन तंत्र के साथ एबीडीएम नेटवर्क पर साझा किया जाता है।

क्या साझा किए गए रिकॉर्ड हमेशा डॉक्टर के पास रहेंगे?

नहीं, रिकॉर्ड साझा करने से पहले देखने की अवधि को तय किया जा सकता है। उपयोगकर्ता द्वारा अपनी सहमति देने के बाद ही रिकॉर्ड डॉक्टर को दिखाई दे सकते हैं।

हेल्थ आईडी को स्वास्थ्य सुविधाओं से कैसे जोड़ा जा सकता है?

• PHR एप्लिकेशन में “लिंक” बटन को सर्च करके और क्लिक करके लिंकिंग की जा सकती है।
• उपयोगकर्ता स्वास्थ्य सुविधाओं पर प्रदर्शित क्यूआर कोड को भी स्कैन कर सकते हैं जो स्वास्थ्य सुविधा के साथ स्वास्थ्य आईडी / पीएचआर पते को स्वचालित रूप से लिंक और प्रमाणित करेगा।

क्या मैं अपने स्वास्थ्य रिकॉर्ड को स्वयं लिंक कर सकता हूं?

हां,

• हेल्थ रिकॉर्ड्स (पीएचआर) एप्लिकेशन में लिंक करने की सुविधा दी गयी है जहाँ क्लिक करके उपयोगकर्ता रोगी अपने स्वास्थ्य रिकॉर्ड को लिंक कर सकता है।
• जब भी कोई व्यक्ति अस्पताल जाता है और सहमति देता है तो उसके हेल्थ आईडी में वहाँ का स्वास्थ्य रिकॉर्ड लिंक हो सकता है।

पासवर्ड भूल जाने पर क्या करें?

• आप मोबाइल ओटीपी या आधार लिंक्ड मोबाइल ओटीपी, जैसा लागू हो, के माध्यम से अपनी स्वास्थ्य आईडी में लॉग इन कर सकते हैं और एक नया पासवर्ड सेट कर सकते हैं।
• लगातार समस्या होने पर ndhm@nha.gov.in पर संपर्क किया जा सकता है या
• टोल फ्री नंबर – 1800-11-4477 / 14477 पर कॉल किया जा सकता है।

तीन बार गलत पासवर्ड डालने पर क्या करें?

यदि आप पासवर्ड के माध्यम से अपने स्वास्थ्य आईडी खाते से बाहर हो गए हैं, तो आप लॉगिन के लिए प्रमाणीकरण के अन्य तरीकों को आजमा सकते हैं। आपको तीनों तरीकों से 3 प्रयास मिलते हैं। यदि आप 3 विधियों में से किसी से भी लॉगिन नहीं कर पाते हैं तो आपको पुनः प्रयास करने के लिए 12 घंटे तक प्रतीक्षा करनी होगी।

सिस्टम में लॉगिंग के 3 तरीके क्या हैं?

सिस्टम में लॉगिन करने के 3 तरीके हैं:

1. पासवर्ड

2. मोबाइल ओटीपी

3. आधार ओटीपी

मैं अब एबीडीएम में भाग नहीं लेना चाहता। क्या मैं अपनी स्वास्थ्य आईडी हटा सकता हूं?

हाँ, आप अपनी स्वास्थ्य आईडी को स्थायी रूप से हटा सकते हैं या अस्थायी रूप से निष्क्रिय कर सकते हैं।

हेल्थ आईडी को डिलीट करने और हेल्थ आईडी को डीएक्टिवेट करने में क्या अंतर है।

 निष्क्रियता के मामले में,

– उपयोगकर्ता बाद में अपनी हेल्थ आईडी को फिर से सक्रिय करना चुन सकता है।

– निष्क्रिय होने पर, आप निष्क्रियता की अवधि के लिए सभी एबीडीएम आवेदनों तक पहुंच खो देंगे।

– जब तक आप अपनी हेल्थ आईडी को दोबारा सक्रिय नहीं करते, तब तक आप किसी भी स्वास्थ्य सुविधा पर अपनी हेल्थ आईडी साझा नहीं कर पाएंगे।

– जब तक आप अपनी स्वास्थ्य आईडी को पुन: सक्रिय नहीं करते, तब तक आप ABDM नेटवर्क पर अपने स्वास्थ्य रिकॉर्ड साझा नहीं कर पाएंगे।

हटाने पर,

– आपके सभी जनसांख्यिकीय विवरणों के साथ आपकी स्वास्थ्य आईडी स्थायी रूप से हटा दी जाएगी।

– आप भविष्य में अपने स्वास्थ्य आईडी पर टैग की गई किसी भी जानकारी को पुनः प्राप्त नहीं कर पाएंगे।

– आप अपने हटाए गए स्वास्थ्य आईडी के साथ एबीडीएम नेटवर्क पर एबीडीएम एप्लिकेशन या किसी भी स्वास्थ्य रिकॉर्ड तक कभी भी पहुंच नहीं पाएंगे।

सरकार द्वारा मेरे डेटा का उपयोग कैसे किया जाएगा?

डेटा संचालित सार्वजनिक नीति निर्णय लेने के लिए सरकार द्वारा बेनामी रिकॉर्ड का उपयोग किया जा सकता है। हालांकि, किसी भी व्यक्ति की जानकारी और स्वास्थ्य डेटा उनकी सहमति के बिना साझा नहीं किया जाएगा।

क्या मेरी अनुमति के बिना मेरे रिकॉर्ड अन्य डॉक्टरों या स्वास्थ्य सुविधा के साथ साझा किए जाएंगे?

नहीं, आपकी सहमति के बाद ही आपका रिकॉर्ड डॉक्टर या स्वास्थ्य सुविधा के साथ साझा किया जाएगा। आपकी सहमति में आप अनुमतियों को अवधि, दृश्यमान रिकॉर्ड के प्रकार के अनुसार अनुकूलित और संपादित कर सकते हैं।

मैं स्वास्थ्य आईडी कैसे प्राप्त करूं?

आप निम्न द्वारा एक स्वास्थ्य आईडी प्राप्त कर सकते हैं:

– हेल्थ आईडी वेब पोर्टल (https://healthid.ndhm.gov.in/register) पर स्व-पंजीकरण

– गूगल प्ले स्टोर से हेल्थ रिकॉर्ड एप्लिकेशन डाउनलोड करके।

– आप भाग लेने वाली स्वास्थ्य सुविधा में अपनी स्वास्थ्य आईडी बनाने के लिए भी अनुरोध कर सकते हैं, जिसमें पूरे भारत में सार्वजनिक / निजी अस्पताल, सामुदायिक स्वास्थ्य केंद्र, स्वास्थ्य और कल्याण केंद्र शामिल हो सकते हैं।

पेशेंट हेल्थ रिकॉर्ड्स ऐप क्या है?

Like Aadhar Card, Government has launched Health Id Card.  Through it one shall be connected seamlessly to all the Health Services across India.

Why Health Card?

Using a Health ID is the first step towards creating safer and efficient digital health records for you and your family. You can opt-in to create a digitally secure Health ID, which allows you to access and share your health data with your consent, with participating healthcare providers and payers.

Use of Digital Health Record

It will help you and your Doctor and also the stake holders, like insurance company to access your information right from admission to treatment and discharge in a paperless manner. 

How does the Holder of Health Id Card Benefit?

In case of an emergency, one goes to the hospital without taking his reports and prescriptions.  So, the doctor has to consider your case freshly. This causes:

1. Delay in start of treatment as doctor waits for the reports to come.
2. Increased expenditure and pain to the person as he has to get even those examinations done for which he has been examined earlier.

If the doctor is given access to the Health Card, he will:

1. Come to know the line of treatment followed earlier which will help him.
2. Suppose the person has gone in coma or is not able to speak then the nominee may access the Health Card and let the doctor know various details required.
3. If the card holder is allergic to some drugs that too shall be mentioned there so one may not administer those drugs to him.

How do Insurance Company’s Benefit by Digital Health Record?

It is well known that several people are misusing the insurance services by fake admissions.   Such discrepancies may get traced easily reducing the burden of claim of Insurance.  This in turn may help the people as the Insurance Companies may reduce the rate of their premium.

Easy Sign up

You simply have to have your Aadhar number and the mobile to receive OTP.  Even if you don’t have the Aadhar, you may sign up with a mobile number.

What is ABDM?

The Ayushman Bharat Digital Mission (ABDM) aims to develop the backbone necessary to support the integrated digital health infrastructure of the country. It will bridge the existing gap amongst different stakeholders of Healthcare ecosystem through digital highways.

What is a PHR?

A personal health record, or PHR, is an electronic application through which patients can maintain and manage their health information (and that of others for whom they are authorized) in a private, secure, and confidential environment.

How can I get my health records from the hospital to my application/phone?

By searching and linking with the health facility you can fetch your health records from the particular facility on the mobile application.

Can I manually upload my health records to the health records application?

Yes, the user can upload their scanned reports to ABDM compliant health lockers via health records (PHR application).

Is all my health-related information going into the health records application?

No, only the records created and uploaded by the doctors and the user themselves will be on the mobile application.

Is the data uploaded on the application secure?

The patient will be the owner of all the data, he/she will have all the rights to manage it. The health lockers integrated with ABDM can also be used for keeping the health records secure in the lockers.

 

Can my health care providers access my PHR to coordinate my health care?

Yes, but only after taking consent from the user, without the user’s consent no-one can access the health records..

 What is a PHR Address ?

PHR (Personal Health Records) Address is a self declared username that is required to sign into a Health Information Exchange & Consent Manager (HIE-CM). Each Health ID will require linkage to a consent manager to enable data sharing. Currently, all Health ID users can generate their own PHR Address during Health ID sign up.

 Can I create multiple PHR address through one Health Records Application?

Yes, multiple PHR address can be created by using one mobile number. And for creating unique health IDs Aadhaar number can be entered which will also make health ID creation easier as it will autofill some field of the form.

What is HIE – CM ?

Health Information Exchange & Consent Manager (HIE-CM) refers to consent manager that enables consent management, and sharing & linking of personal health records for a user. ABDM has rolled out its own HIE-CM (Health Records Application). You can use your Health ID to sign up on the HIE-CM (Health Records Application). Multiple consent managers are likely to be available for patients to choose from in the near future.

What all do I need to register for a Health ID ?

• For Health ID creation through Mobile Number- Name, Year of Birth, Gender, Address, Mobile Number.
• For Health ID creation through Aadhaar- Name, Year of Birth, Gender, Address, Mobile Number, Aadhaar Number

Is Aadhaar mandatory to create a Health ID ?

No. The process of generation of Health ID involves voluntary usage of Aadhaar. If you do not want to use your Aadhaar for Health ID creation, you can simply use your Mobile Number accompanied with self declared information.

My Mobile Number is not linked to my Aadhaar Number. How do I authenticate my Aadhaar for Health ID creation ?

If you use your Aadhaar Number to create your Health ID on the Health ID portal, an OTP will be sent to the Mobile Number linked to your Aadhaar. If you are not having the mobile linked to Aadhaar, it is suggested that you visit the nearest participating facility, and opt for biometric authentication using Aadhaar Number. After successful authentication, you will obtain a Health ID at the facility itself.

Can I use ID documents other than Aadhaar for Health ID creation ?

Currently, ABDM supports Health ID creation via Mobile or Aadhaar. ABDM proposes to   soon roll out features that will support Health ID creation with other ID documents such as PAN card, Driving License, etc. in assisted mode at participating health facilities.

Is my Health ID unique?

Your Health ID is created by using your basic details in addition to your Mobile Number or Aadhaar Number. Your Health ID will be unique to you, and you will have the option to link all your health records to the Health ID.

Can I Create Multiple Health Id so that I may hide some of my problems with Family Doctor or someone known to me?

You can choose to create multiple IDs to link different sets of health records with different Health IDs, however, it is recommended that you create only one Health ID.

Why is ABDM allowing a user to have multiple health IDs ?

ABDM associates high value with a user’s privacy. ABDM believes that users are rightful custodians of their data, and can make rational decisions associated with its use. Given that healthcare is a sensitive subject, ABDM does not want to restrict users from linking different sets of health data with different Health IDs. For instance, if a user wants to use a separate Health ID to access data related to their sexual history, ABDM will allow for such use.However, in order to establish a better continuum of care, it is recommended that users create & use only one Health ID. A unified longitudinal health history linked to a single Health ID can lead to informed healthcare decisions for a user.

How long will the registration take?

Complete registration takes few minutes as you are only required to fill your basic details,an authenticate your Mobile Number or Aadhaar Number.

Do I have to submit any physical documents?

As part of the existing process, you are not required to submit any physical documents anywhere.

Are my health records safe and secure?

ABDM does not store any of your health records. Your health records are stored with healthcare information providers as per their retention policies, and are shared over the ABDM network with encryption mechanisms only after your express consent.

 

Will the shared records stay with the doctor forever?

No, viewing duration can be customized prior to sharing the records. The records can only be visible to the doctor once the user has given their consent.

How can health ID be linked with the health facilities?

By searching and clicking on “link” button in the PHR application linking can be done. Users can also scan the QR code displayed at the health facilities which will automatically link and authenticate health ID/PHR address with the health facility.

 Can I link my health records on my own?

Yes, user can link their health records using patient initiated linking which is a functionality provided in Health Records (PHR) application. Health records linking can also happen at the health facility whenever an individual visits the hospital and creates Health ID, post individual’s consent.

What do I do if I forget my password?

You may login to your Health ID through Mobile OTP or Aadhaar linked Mobile OTP, as applicable, and set a new password.

What to do if an incorrect Password is entered three times?

If you have been locked out of your Health ID account via Password, you can try other modes of authentication for login.  You get 3 attempts by all the three methods.  If you could not login by any of the 3 methods you will have to wait for 12 hours for trying again.  

What are the 3 methods of logging in the system?

The 3 methods of login in the system are:

1.  Password
2. Mobile OTP
3. Aadhaar OTP

I no longer want to participate in ABDM. Can I delete my Health ID ?

Yes you may either permanently delete or temporarily deactivate your health id.

What is the difference between deleting the Health Id & Deactivating the Health Id.

 In case of deactivation,

• a user can later choose to reactivate their Health ID.
• On deactivation, you will lose access to all ABDM applications for the period of deactivation.
• Until you reactivate your Health ID, you will not be able to share your Health ID at any health facility.
• Until you reactivate your Health ID, you will not be able to share your health records over the ABDM network.

On deletion,

• your Health ID will be permanently deleted, along with all your demographic details.
• You will not be able to retrieve any information tagged to your Health ID in future.
• You will never be able to access ABDM applications or any health records over the ABDM network with your deleted Health ID.

Anonymous records can be used by the government to make data driven public policy decisions. However, no individual’s information and health data shall be shared without their consent.

 Will my records be shared with other doctors or health facility without my permission?

No, your records will be shared with the doctor or health facility only after your consent. In your consent you can customize and edit the permissions in terms of duration, type of records visible.

How do I get a Health ID?

You can get a Health ID by:

• self-registration on the Health ID web portal (https://healthid.ndhm.gov.in/register) 
• by downloading Health Records application from the Google Play store.  
• You may also request for creation of your Health ID at a participating health facility, which may include public/private hospitals, community health centre, health and wellness centres across India.

What is Patient Health Records App?

The Patient Health records App allows a patient to view their health data from the different hospitals, labs and doctors in one place. You can download reference patient Android App. Download .apk file

How can I edit my mobile number in my health ID card of NDHM?

For editing the mobile number one should go to  the site  https://healthid.ndhm.gov.in and click on the login.  When you login to your page, on the left side you can see the heading My Account.  In that you will get the heading Update mobile number.  You have to simply enter the new mobile number and fill the OTP received on the new mobile number.  It will get updated.  That’s So Easy.
